heartworm preventive is an important part of any dog's life, ANYwhere heartworm is endemic - which is now the vast majority of the U-S [it was once a Midwestern problem].

Myth: ‘Indoor’ dogs can’t get heartworm - Web Only Article | Tufts Your Dog

It only takes ONE mosquito-bite to transmit microfilaria - please don't throw dice with Ur dog's health; U can shorten their lifespan, & make their lives unhappy with exercise restrictions, or when treated with a heavy parasite load, they can throw a clot / stroke, have a blockage & die, or have other major complications.
Killing spaghetti-like worms in the dog's heart is a risky prospect.

I'm not even sure why that's a myth - how many dogs are truly indoor-only anyway?

We were never terribly consistent with heartworm preventative either until my parents brought Nellie home. She was already infected from her prior home and had to go through the expensive and emotionally taxing treatment. That was a major wake up call - now all dogs are on heartworm meds and always will be.

there are a few, but rare, really.

...
yup - i've known a very-few, apt-dogs or condo-dogs, often belonging to elderly ppl,
some were trained to use potty-pads in the bathroom, others to void in a walk-in shower, which actually works quite well -
solids go down the toilet, urine down the drain, scrub & hose, et voila! - All clean.

The vast majority of dogs spend hours outside daily - where U are determines which Mozzie species are local, & when they bite / are active.
How that coincides with an individual dog is whatever the owner chooses, on a given day. It could be anything - i've known owners who worked swing shift, & took the dog out to run at 11-pm some days, or 4-am on others.
Whatever works. :D
