Since you're dealing with an injury, you might want to actually consult a Canine Rehabilitaion/physical therapy specialist. Just to make sure exercises are safe and designed for your dog's needs!

You might find one near you or find one to consult with online or via phone through organizations like this.

Also Debbie Gross offers online canine conditioning classes through the Fenzi Academy. I think her class this next session is Level 2 but something to check out. I think she also does phone consults....
