Rear end issues!

GRAPHIC PIC :/ - I have an old dog with some new things going on with his behind. I'm worried and other people's posts don't seem to cover this...
I understand he is old.16 or 17 this year. I've had other dogs and I'm no stranger to old dog problems but I've never seen something like this and I'm worries he is suffering while I could be doing something for him.

I have no idea what I am supposed to be looking at here but I suspect a trip to the vet would be most appropriate.
