i'd see another vet - or return to the same one, whichever - & ask them to do a skin-scraping, & put it under the 'scope.
Sarcoptes, Demodex, ringworm [75% of Ringworm fungi fluoresce under UV light / a blacklight in a dark room], yeast explosion, contact dermatitis, ______ ?? - who knows, it could be any of those, or something else.
U need more data - only a vet can provide that. A vet-dermatologist might be an excellent idea; U can ask the GP vet for a referral, or a derm-specialist at a university Vet-College might help, using this case as a teaching tool, which could save $$$.
