After searching for a good bed for weeks, and sorting out what we really wanted for the dog, we ended up with this:
We bought a baby mattress (120 cm x 60 cm x 12 cm) and we made a bed frame out of wood, so said mattress wont be directly on the floor to avoid moisture problems and to be able to clean under it.
Put the mattress on it, add a "alèse" (I don't how to say this in english, it's something you put between mattress and sheet for children mostly so they wont wet the mattress if they have an accident as they sleep), and a fitted sheet.
Wash the sheet and "alèse" when needed, replace them if ruined for very cheap.
For small/medium dogs I use bed pillows - covers are easy and cheap to come by. They seem to prefer them to the expensive dog-specific versions.
Borg - love the dog in your avatar! For my karakachan I cut a piece of 10cm thick foam and made washable covers from furry fabric I bought in the 2nd hand shop. He loved it, he used to drag it around the garden to find the best sunbathing spot.

But, probably, also I'll have to add some step, what can enable him to reach the bed

All of the beds have removable (washable) covers. We found that our GS gets her bed pretty darn smelly and that it's HUGE for us to be able to wash the covers.
They're also available in a variety of sizes, as well as waterproof or fleece materials
