4 month puppy speration anxiety

We need some major help!! Our puppy has some terrible anxiety and we can not leave her for more than 15minutes with out her peeing or pooping on herself. Here is our progression:
-picked up at 8months
-used very small crate when we went to work from 9-5 and had a dog sitter come at 1 to let her out to go to the bathroom.
-got good at not going in our crate
-then got a UTI and started peeing in our crate which led to her thinking it was okay to poop
-wife stopped working now as she is going back to school soon but we still need to be able to leave sometimes
-tried a new bigger crate with a playpen around it to start so she could come and go comfortably from the crate. Put pee pads in the pen to try to avoid peeing in the crate
-jumped the 3.5' playpen and got out so need to go to the next step
-just puppy proofed a room 4walls and her crate with door open.
-sat with her in the room for an hour playing with her and getting her used to it
-left her in the room for 30minutes as we sat quietly down stairs so she couldn't hear us and she barked and cried for the whole time
-went back up when she was quite for a minute and opened the door calmly
-again she pooped and peed all of the room and was traumatized again.

What do we do? The puppy is very well stimulated and exercised. She listens and completes commands very well like sit stay lay paw come. But as soon as we aren't around she is a different dog. Pleas help we don't know where to start we feel everything got ruined when she got her UTI.

I've found that my dog, Sadie, has some separation anxiety problems, too. However, she is a full grown Rottweiler/Shepherd Mix that weighs 90 lbs! I don't think she has it quite as bad as your puppy, but again, she is older. Puppy pads are a very bad training technique.

I've been taking Sadie on a 40-minute walk once a day. I train her while we are walking with treats. I've given her a lot of toys to play with while I'm gone. She has many chew toys and large rawhide bones. Because she barks very loud and for hours when I leave (my neighbors have complained), I've had to use a bark collar on her. I've also gotten her on a prescription anxiety medication to calm her down. I leave the television on for her, too. My veterinarian told me that some rescue dogs develop anxiety problems because of abuse, neglect, and/or abandonment and sometimes they need medication to help them.

Is she still going in the crate when you put her in there? (From a previous post)

My other thought is only contain her alone after a walk when she is tired so hopefully she sleeps

It's ok if she barks a bit, maybe start small, like 10 minutes
